THE ENROLLMENT PROCESS
STEP ONE –
1. If you are a non-scholarship family, enroll at Incredible Horizons as soon as possible to secure your placement. A payment contract and deposit will be required. Call 321 751-1313 for appointment
2. If your child has been
assigned an Individual Education Plan through the public schools, apply for
the John McKay Scholarship by filing a letter of intent by July 3 at
www.floridaschoolchoice.org

then next screen click on "Apply
For Mc Kay Scholarship" on the left side bar
Print and save
several copies of the screen documenting that you submitted the letter of
intent confirming your eligibility-keep them in a safe
place. (If in doubt print the screen)
You will need to bring a copy to Incredible Horizons ASAP to ensure opening for
your child.
***You can also come in and have the admissions specialist assist you in
filing the
intent so the confirmation letter is put directly in your child's
file.

3. If your child is already receiving the John McKay Scholarship, simply withdraw from your current school and let us know that you desire to transfer their scholarship to Incredible Horizons Academy. Call 321 751-1313 if you need help.
STEP TWO –
Provide the following information to the Incredible Horizons office:
· Application for Admission
· Appropriate Fees and Tuition
· Health and Immunization Records (see back of page for requirements)
· Copy of Birth Certificate(s)
· Most Recent Report Card and IEP

Complete the following:
· Physical Examination
· Eye Examination-including convergence testing
· Educational Evaluation for curriculum placement
· Admission Conference
· Parent – Student Contract
We often ask parents to go
through a process of ruling out physical issues that may relate to their child's
learning disability. We have found that there are many physical ailments that
interfere with learning. Discovering that there is something physical that can
be treated can make a huge difference in a child's performance in school.
Ask your physician to test anything that may contribute to the
symptoms-Rule out: Seizures, Brain Lesions, Neurological disorders, Genetic
disorder, Metabolic Disorder, Thyroid issues, Allergies, Toxicity etc. This can
be scheduled along with your physical exam.
Check Convergence & Visual perceptual issues that may affect reading
should be tested by a pediatric developmental ophthalmologist- Dr. Thomas

If concerned about Motor control or motor planning problems,Sensory
Integration problems, or visual motor problems- Call for an evaluation at
Child and Family Consultants 321 768-6800 (Takes many insurance policies) or

Toxicity and nutritional issues can be tested and treated at Total Life

If
anyone in your family has sugar issues, diabetes or hypoglycemia, the
endocrinologist that does a more in depth study is Dr. Mauri Cartakushansky (Dr.
C) at Nemours in Orlando. 407 650-7210.
He usually asks students to be put on a 4-hour stimulant for ADHD so the
children eat lunch. This way students are not experiencing low blood sugars
while they are coming off the medicine in the afternoon. He also recommends that
children are not given pop, Gatorade or sports drinks.
This sounds odd but when the vertebrae are out of alignment, nerves become
stretched or twisted and the vital communications in the brain and CNS are
disrupted. This neurological disturbance is called subluxations. This can
lead to all kinds of neurological, emotional, and physical disorders, Cognitive
disorders and ADHD. For structural problems with CNS see Dr. Sal

We believe the whole child needs to be treated in order for any child to learn to their maximum potential. We like to know that the child is functioning at his optimum physically before assembling an educational intervention plan.
Immunization Requirements
First-graders:
1. Must be 6 years old on or before September 1st
Any student enrolling at a Florida school for the first time must have proof of the following immunizations:
1. DPT (diphtheria, tetanus, pertussis), 5 doses
2. Polio, 4 doses
3. Measles
4. 1 Varicella (or parent verification of Chicken Pox), 2 doses
5. Kindergartners must have hepatitis B, 3 doses
All seventh-graders must have proof of the following
immunizations:
1. Hepatitis B, 3

2. Measles vaccine, a second dose
3. Tetanus-diphtheria booster
